CHHS Medical and Religious Exemption Process for Clinical Requirements
CHHS Medical and Religious Exemption Process for Clinical Requirements
Students seeking a medical or religious exemption must submit supporting documentation through Med+Proctor.
Medical exemptions must include provider-signed documentation that explains the medical basis for the exemption request and identify the specific requirement(s) for which an exemption is being sought.
Religious exemptions must include a written statement describing the religious basis for the request and identify the specific requirement(s) for which an exemption is being sought.
Med+Proctor will review the submitted documentation and approve the exemption request, if appropriate.
Students will receive a confirmation message in Med+Proctor and an email sent to your zID email address indicating the exemption has been accepted.
Students must save proof of approval (e.g., screenshot, saved document or official confirmation message).
After approval, students must upload the following to EXXAT (This step is required for program tracking and clinical compliance review):
Med+Proctor approval (screenshot or official email message).
Supporting exemption documentation.
The Administrative Aide will review submissions in EXXAT to verify:
Required documentation is complete.
Med+Proctor approval has been properly documented.
Will confirm accuracy of exemption with the Immunization Office if needed.
The Administrative Aide will send formal written notification to the student including:
Approved final exemption status on file.
Any clinical site restrictions or requirements associated with the approved exemption.
