Applicant Instructions

NIU Admission

Prior to applying to the Medical Laboratory Sciences program at NIU, the applicant must first be admitted into the university. The MLS application requires a Z-ID in order to complete the form which is provided by NIU after successful admission into the institution. Once a Z-ID is obtained, an application can then be submitted into the MLS program which allows all potential applicants to begin the application process.

Letter of Recommendation Form

Download the Letters of Recommendation document. These must be filled out by two professors/supervisors. For the signature portion of the form, an electronic signature is accepted. Otherwise, type your name in the signature field, save it and send it to your evaluator via email attachment.

Application Form

Download the Application form to fill out the MLS application. This particular form requires a personal goal statement, general student information, academic information, and the essential functions form. After completion, it can be saved and sent digitally to Prospective students should submit their formal application to the medical laboratory sciences program by February 1 of their sophomore year.  Additional applications received by May 1 are considered based on space availability.

Acceptance Time Frame

All accepted students into the MLS program will begin their program in the fall semester of their acceptance year. All accepted MLS students must attend a mandatory orientation session the Friday before the start of the academic fall semester. A specific date will be given to you upon acceptance.

Accelerated Degree Completion (ADC) Students

ADC students are allowed to begin their studies in fall or the spring semester upon request.
